Notarial Record Form for Signing Agents in Cook County, IL

For your convenience, USNA is providing a downloadable copy of the Notarial Record – Residential Real Property Transactions form. Beginning on June 1, 2009, this form will be required by the state when real property in Cook County is sold or conveyed.

Notary signing agents must look at the legal description of the property in the document of conveyance to verify that the property is actually located in Cook County. If so, then a notarial record must be filled out.

The notarial record is not kept by the notary but delivered to the title insurance company or agent, financial institution, attorney employing the notary, or to the Cook County Recorder of Deeds Office within 14 days of the notarization.
